Assessment task: Community Profile

*Intent:

Provides students with the opportunity to understand, collect, analyse and succinctly provide data and information relevant to health planning.

*Task:

This is the major assignment for this subject. It requires students to explore a specific region and/or population group. Accurate identification, presentation, critical analysis and evidence based interpretation of key data to reliably inform health service planning is required. A comparison of the population and/or region under investigation against other population groups is also required.
The subject coordinator will provide details of the specific population group and/or region to be investigated during on-campus day 2. In-class sessions on this day will provide knowledge and skill development in data identification, interrogation, presentation, analysis and interpretation.
A written assignment guide will be made available to students for this assignment and it is highly recommended that this guide be carefully followed to ensure its successful completion.

2. Compare findings against the NSW state average. Analyse and interpret your findings providing sound evidence-based rationale for any differences found.
3. Locate and select the lowest level data available. Locate the data for the relevant locations and paste into a new Excel spreadsheet.

Identify and graphically illustrate:

o Numbers and rates of older persons in each region
o Numbers and rates of existing avoidable presentation and admission rates in older persons
o Numbers and rates of at-risk populations for avoidable presentations/admissions in older persons
o Numbers and rates of at-risk conditions for avoidable presentations/admissions in older persons
