Respiratory illness with a previous diagnosis of diabetes

Introduction:
Provide a brief introduction to the case :
*use a pseudonym for your patient
*presenting problems
*medical diagnosis.

Include:
*summary of the patient’s history (past medical and social history) *presenting signs and symptoms *relevant diagnostic and pathology test results
*current medications

Problems:
*Identify the signs and symptoms that you believe to be altered for your patient, and prioritise these according to clinical urgency using the A-G format.
*A-E is satisfactory.

Pathophysiology:
*Discuss the pathophysiology underlying the altered signs and symptoms, with reference to the patient’s disease process. Within this section you need to demonstrate a comprehensive understanding of the relationships between the patients’ signs and symptoms, and the underlying cellular AND systemic pathophysiological processes.

General considerations:
– Look at the allocation of marks – this will show you where you should be focussing your effort/word limit.-Abbreviations – write in full first time, then abbreviate
– Subheadings are appropriate in this paper if you wish to use them
– References – need to be recent (last 5-10 years)

Criteria:

Identification of significant underlying pathophysiological process impacting your:

1) patient, and provision of a comprehensive patent history (20%)
2) Identify and prioritise the altered physical findings (20%)
3) Discussion of cellular and systemic pathophysiology underlying the altered signs and symptoms (60%)

Marking Rubric :

1. Introduction: The student is able to provide a brief specific introduction to the case study which includes an accurate identification of the significant underlying pathophysiological process impacting the patient, and a comprehensive patient history.
2. Problems: The student is able to clearly identify all the patient’s altered physical findings and prioritise these findings according to clinical urgency using supporting evidence-based literature.
3. Discussion – Discuss the pathophysiology underlying the altered physical findings with reference to the patient’s condition: Within this section the student is required to demonstrate a comprehensive understanding of the relationships between the patients’ physical findings, and the underlying cellular AND systemic pathophysiological processes using evidence-based literature.

Assessment: Case study

Select a case study from your workplace. If you are not currently working, you may use a patient that you have previously cared for. This patient must be experiencing 2 significant health issues. Examples may include a person with a respiratory illness with a previous diagnosis of diabetes, or a person with cardiac issues with a previous diagnosis of dementia.

1. Introduction -Provide a brief introduction to the case (remember to use a pseudonym for your
patient) which includes presenting problems and medical diagnosis. Include a summary of the
patient’s history (past medical and social history), presenting signs and symptoms, relevant
diagnostic and pathology test results, and current medications.

2. Problems – Identify the signs and symptoms that you believe to be altered for your patient, and
