Patients need enough information on how to continue advancing their health condition
after being discharged from the hospital. The brochure provides significant and relevant
information on the processes and steps that a patient can take to understand their condition. The
primary consideration of the brochure is to enhance speaking up among patients. Significant
points provided are about finding out further about one’s condition and the necessary steps to
take to after being discharged. Additionally, the pamphlet guides asking about medicines for the
patient and the follow-up to improve after discharge care. Significantly, the brochure encourages
patients to communicate about doctor visits, home care, community service, and hospice. The
brochure elaborated on the need for patients to communicate with their health providers to ensure
the continuous provision of health care services.
Patient Communication
Patient communication presented in the brochure is significant n outlining how the
patient is expected to interact with the health care providers. Patient communication, in this case,
involves the patient asking questions and receiving responses. The patient needs to have a
notebook where they outline the responses provided. More so, the patient will expect elaboration
regarding the treatment and medication they have received in the facility and any further care
needed. According to the brochure, a nurse or doctor is responsible for enhancing effective
patient communication. As a result, the patient will leave the hospital with enough information
regarding their health.

I found the brochure very useful and important for patients leaving hospitals. I believe
