notes are the foundation ofany report, andit is imperative to have them when writing an incident

report or a supplemental report. Taking note of what you observe helps you to familiarize with

the event and makes it easy to remember the facts of a case. I learned that field notes should be

sufficientto provide all the information you need. Another valuableinformation I learned from

this class is that for a report to be complete, you have to include all the elements such as who,

what, where, when, why, and how. The users of the report, for instance, need to know who

reported the occurrence, the area and address it took place, what happened, why it happened,

how the incident ensued and when the incident was discovered. All these questions should be

answered in an easy to read method (Hess & Orthmann, 2008
